There are two different payment opportunities when you work with Well Rooted Family Practice. You can pay per visit or join our family with a membership. We are a cash only clinic and do not accept insurance.

*Each adult membership is a $75 one time start up cost. Each child membership is a $50 one time start up cost.

*Minimum time requirement for each membership is 6 months. Can cancel anytime after 6 months, with a 30 day notice.

*Memberships include all well visits for children, physicals for adults, one monthly visit per person, and a discounted rate on labs, other appointments, and additional services.
