There are two different payment opportunities when you work with Well Rooted Family Practice. You can pay per visit or join our family with a membership. We are a cash only clinic and do not accept insurance.

Per Visit Pricing
We offer a variety of visit types based on your needs.
Adult Physical

*For new patients, there is a one time fee of $75 for adults and $50 for children. Also, expect up to an additional 30 minutes for your first visit for your provider to get a thorough history and intake.
*These prices do not include the cost of labs, tests, or recommended treatments.

*Each adult membership is a $75 one time start up cost. Each child membership is a $50 one time start up cost.
*Minimum time requirement for each membership is 6 months. Can cancel anytime after 6 months, with a 30 day notice.
*Memberships include all well visits for children, physicals for adults, one monthly 30 minute visit per person, and a discounted rate on labs, other appointments, and additional services.
